How is The Choice Care Card™ used?
When you visit the doctor, present your healthcare plan ID card. If the plan has a co-pay, use The Choice Care Card™ to make the payment at the time of service. If your plan subjects the office visits to a deductible, use The Choice Care Card™ to make your payment once the provider bills you.
The Choice Care Card™ makes the payment to the provider from the fund established by the employer. Account balances are accessed online. Annual statements are also made available to employees.
Of course, claims may also be submitted manually. If an eligible claim is submitted via mail or fax, Choice Care Card will mail the employee a check or direct-deposit the funds into his or her account – whichever is preferred.
